Acknowledgment of Risk and Consent to Participate

I, , hereby acknowledge that participation in fencing training and related physical activities at Silver Blade Fencing Academy LLC involves inherent risks, including but not limited to minor injuries such as bruises and sprains, and rare but serious risks such as concussions, broken bones, or other physical harm.

I voluntarily assume all such risks on behalf of myself or my minor child and consent to participation in programs, classes, lessons, camps, and events offered by Silver Blade Fencing Academy LLC.

I acknowledge and understand the physical risks involved.

Medical Release

In the event of injury or medical emergency, I authorize Silver Blade Fencing Academy LLC staff to secure appropriate medical care. I understand that every effort will be made to contact the emergency contact listed above.

I authorize emergency medical treatment if necessary.

Release of Liability

I hereby release, discharge, and hold harmless Silver Blade Fencing Academy LLC, its owners, coaches, assistants, employees, and volunteers from any and all claims, demands, actions, or causes of action related to injury, illness, or loss arising out of participation in any activity at or affiliated with Silver Blade Fencing Academy LLC.

This release applies even if such injury results from negligence, provided it is not due to gross negligence or intentional misconduct.

I release Silver Blade Fencing Academy LLC from liability related to participation.

Photography and Media Release

I consent to the use of photographs or videos taken during class or events for promotional purposes (e.g. website, social media).

Equipment Rental Agreement

Silver Blade Fencing Academy LLC provides loaner fencing equipment (including masks, jackets, gloves, and weapons) for up to one month to new students.

By signing this agreement, I agree to the following terms:

• I will return all loaned equipment in clean and good condition by the agreed return date.

• I understand that damaged, lost, or unreturned items may result in a replacement fee.

• I agree to treat all rented equipment responsibly and store it safely when not in use.

I agree to the equipment rental terms and accept responsibility for any damages.

Agreement for Minors

If the participant is under 18 years old, I certify that I am the legal parent or guardian of the minor named above, and I grant permission for their participation in all fencing-related activities and equipment use.
